Do you know anyone who is on AHCCCS, uninsured, underinsured, and has experienced challenges getting dental care? We are looking for Arizonans who struggle to get dental care to complete a short online survey.
The CAA/AZOHC works with a dedicated group of advocates with oral health lived experience, and this group has developed this survey. The group and the AZOHC would like to thank you and your organization for your support in this initiative aimed at enhancing access to dental care for the people we serve. We kindly request that you distribute a short survey to your staff and clients, encouraging them to share their experiences with dental care.
